CHOICE Arrangement costs and pricing

The largest cost is the employer-funded allowance. Setup, administration, enrollment support and optional benefits should be shown separately so employers can evaluate the complete budget.

Four cost layers

  1. Employer allowances.
    Monthly amounts available for employees, spouses and dependents.
  2. Administration.
    Eligibility, substantiation, reimbursement, reporting and ongoing plan support.
  3. Arrangement setup.
    Contribution modeling, documents, notice preparation, implementation and launch support.
  4. Optional services.
    One-to-one enrollment, ancillary benefits, HSA support, compliance services and specialized analysis.

Choice Benefits annual service levels

  • Starter: 2–15 employees — $750
  • Launch: 16–25 employees — $1,500
  • Growth: 26–50 employees — $3,500
  • Enterprise: 51–100 employees — consultation and quote
  • Custom: 101+ employees — consultation and quote

Final pricing depends on scope, employee count, service model and selected partners. Insurance premiums and administrator charges are separate from Choice Benefits service fees unless a proposal says otherwise.
