This is a fixed snapshot of DOL’s 2025 form-year “Latest” files, dated September 25, 2026 and retrieved October 15, 2026. It includes welfare-plan filings with a Schedule A medical or stop-loss indicator in that release. Only those medical and stop-loss insurance records are displayed, with broker records joined by filing acknowledgment ID and schedule ID. Where the same employer EIN, plan number and plan period have multiple records, the greatest acknowledgment ID is retained. Form year may differ from the actual plan period shown in each result.

Amounts are reported historical Schedule A commissions and fees; they are not a complete accounting of every advisor’s compensation. Standalone dental, vision, life and disability contracts, Schedule C and other compensation sources are not included. A blank amount means not reported, not zero. Covered persons and plan participants are different measures. Combined contracts may include several benefit types. Missing results do not establish that an employer lacks coverage or failed to file. Public filings cannot establish current coverage, claims risk, future renewal rates or ICHRA suitability.
